Advances in the Management of Thyroid Pathology and Obesity
The King Juan Carlos Hospital hosts the 'Update Conference on Endocrinology and Nutrition, Obesity and Thyroid Pathology' to analyze advancements concerning these two pathologies.
The Rey Juan Carlos University Hospital in the Community of Madrid celebrated the 'Update Day in Endocrinology and Nutrition, Obesity and Thyroid Pathology' with the goal of updating knowledge on these two pathologies.
"Both thyroid diseases and obesity now have increasingly effective diagnostic tools and treatments." This is the main conclusion conveyed by Dr. Nerea Aguirre Moreno, associate head of the Endocrinology and Nutrition Service at the Rey Juan Carlos University Hospital, during the event. The main goal of the educational meeting, organized by the aforementioned service, was "to update knowledge on two very prevalent pathologies: thyroid diseases and obesity, aimed at all healthcare professionals, including Primary Care physicians, who constitute the first level of care and a fundamental pillar of our health system," continues the specialist who emphasizes the importance of "always consulting healthcare professionals for any questions, avoiding unverified information, and opting for prevention, early diagnosis, and proper follow-up to improve the health and quality of life of patients."
For this purpose, the 'Update Day in Endocrinology and Nutrition, Obesity and Thyroid Pathology' was structured into four areas dedicated to thyroid pathology, clinical evaluation and new therapeutic strategies in obesity, obesity in specific clinical situations, and key topics in clinical practice; addressing the main diagnostic and therapeutic challenges, from interpreting an altered TSH (thyrotropin, thyroid-stimulating hormone) or managing a thyroid nodule to new pharmacological strategies for treating obesity, its relationship with specific clinical situations, and the need to maintain long-term weight loss achieved.
Thyroid Pathology
The first part of the day focused on thyroid pathology in clinical practice, addressing issues such as the interpretation of TSH alterations, the management of thyroid dysfunction, and the keys to interpreting thyroid nodule ultrasound.
One of the most frequent concerns in consultations, according to specialists, is patients' worry about the possible malignant nature of thyroid nodules. However, the vast majority of these nodules are benign. Currently, ultrasound classification systems allow for a very precise assessment of malignancy risk and decide which patients need further studies or follow-up, avoiding unnecessary tests and improving care.
In this regard, Dr. Ana Alonso Torres, from the Radiodiagnosis Service and the Section of Neuroradiology at the Hospital, highlighted the importance of a multidisciplinary approach, especially in the coordination between Radiodiagnosis, Endocrinology and Nutrition, and other involved specialties.
Obesity
The meeting also dedicated a significant part to obesity, both from the perspective of its clinical evaluation and new therapeutic strategies; and specialists insisted on addressing it as a chronic and complex disease, not just a weight issue.
Obesity is associated with numerous health complications, including type 2 diabetes, high blood pressure, cardiovascular disease, fatty liver, some types of cancer, and osteoarticular problems. Additionally, biological, genetic, psychological, social, and environmental factors are involved in its onset and maintenance, necessitating a comprehensive approach that considers not only excess weight but also associated diseases and the specific circumstances of each patient.
In this context, new pharmacological therapies have represented a significant advancement. Until a few years ago, the available options offered more limited results, while current treatments allow for clinically significant weight losses, which can be between 15 and 20 percent of body weight in many patients. However, specialists remind that these are medical treatments that must be prescribed and supervised by healthcare professionals, are not indicated for everyone, and should always be accompanied by lifestyle changes.
The program of the day also addressed specific clinical situations related to obesity, such as menopause, metabolic liver disease, sarcopenic obesity, or cardiovascular risk. All of these require specific assessments, as they can condition both health risks and therapeutic decisions.
During menopause, for example, changes in body composition and fat distribution can increase cardiovascular risk. Metabolic liver disease associated with obesity can progress silently, so early detection is crucial.
Sarcopenic obesity combines an excess of body fat with a decrease in muscle mass and function, increasing the risk of frailty, disability, and functional impairment. In these cases, in addition to weight loss, it is especially important to promote strength exercise adapted to each person.
Another central message of the meeting was the importance of maintaining weight loss. While losing weight is relevant, maintaining that loss long-term is the real challenge because the body tends to defend the previous weight through biological mechanisms that favor weight regain, thus obesity must be understood as a chronic disease requiring treatment, ongoing monitoring, and professional support.
In this regard, maintaining healthy eating habits, engaging in regular physical activity, taking care of sleep, addressing emotional factors, and having healthcare support are fundamental elements to consolidate the results obtained, the experts reminded.
Finally, the meeting highlighted the importance of coordinated care between Primary Care, Endocrinology and Nutrition, Radiodiagnosis, Internal Medicine, Nutrition, and Mental Health. This collaboration allows for more complete, personalized, and patient-centered care, with more accurate diagnoses, better coordination of treatments, and more effective support throughout the care process.
